How to Prepare for a Visit?
Preparing for your Visit
We need to clearly and completely understand your medical history and any treatment you may have had in order for you to safely participate in a study.
It is important to bring the following items to your visit:
Medical Records, including copies of exams such as X-Rays, Lab results and office notes. If you don't have copies of your records, don't worry, we can order them on your behalf just bring your current or previous physicians name and contact information.
A List of Current Medications: This includes all prescription medications, over-the-counter medications, alternative medications, vitamins, herbs and supplements.
Valid photo ID.
List of questions to ask the study coordinator.
We encourage you to bring a family member or significant other to help you take notes and also ask questions.
